This is to inform you that effective June 11, 2020, I will no longer practice medicine with Hamilton Vascular. Carlos Hamilton, MD is available to continue your care at Hamilton Vascular and I recommend Dr. Hamilton to see you as needed. In the meantime, I will continue to treat you until you have had a reasonable time to secure the services of another physician at Hamilton Vascular.
 
Your medical records are confidential and Hamilton Vascular will retain custodianship of your medical records. A copy of your medical records can be transferred to another physician if Hamilton Vascular receives a valid written authorization.
